Verdict

Milan to Rome costs €45.60 in tolls for a car in 2026, and Milan to Naples €62.20.

You take a ticket where you join the motorway and pay where you leave it, by card, cash or a toll device. Cards are taken without a PIN, and every automatic exit lane has a contactless reader.

Drive Milan to Rome and back and the barriers take €91.20, before fuel.

Italy charges by the kilometre, and the rate depends on the section: a mountain stretch costs more than a flat one. The only reliable figure is the operator's own calculator. Every price below comes from Autostrade per l'Italia's route and toll calculator, for a car (class A), read on 13 October 2026. Where a drive crosses another company's motorway, such as the A22 over the Brenner or the A4 from Turin, the calculator adds that company's price too.

Budget €45.60 From Milan to Rome

The A1 runs the length of the country. From Milano Sud to Roma Nord, where the A1 meets the Rome ring road, the toll is €45.60. Carry on to Napoli Nord and it is €62.20 for the whole drive from Milan.

Section by section: Milan to Bologna is €16.80, Bologna to Florence over the Apennines €7.90, Florence to Rome €20.00 and Rome to Naples €16.00.

Roma Nord and Roma Sud are on the motorway ring round Rome, not in the city. Bologna to Rimini is not in the table: the calculator split it into two tickets, so I'd rather leave it out than give a figure that may not match your receipt.

Adds Autostrade per l'Italia's 2026 prices for a car (class A) from the table above. Two drives that meet at a city, such as Florence, can have a short stretch between them that is not in either price.

How to Pay Tolls in Italy: Ticket In, Card Out

Take a ticket from the machine as you join the motorway, and keep it. At the exit you hand it over, or feed it into the machine, and pay for the distance.

Without a box, keep out of the lanes reserved for Telepass customers. Old Viacard prepaid cards are no longer sold, and the credit left on one can be used until 31 December 2029.

Lost the Ticket? You Pay From the Farthest Station

Lose the ticket, or enter where no ticket was issued, and you are charged for the whole motorway, from the station farthest from your exit. Autostrade lets you fill in a self-declaration of where you really joined and ask for the toll to be worked out again.

I'd put the ticket in the same place every time, with the car keys.

Pay an Unpaid Toll Within 15 Days

If you cannot pay at the exit, the lane gives you an unpaid-toll report. Pay it within 15 days and nothing is added. You can pay online from 24 hours after you passed, on the Autostrade site or app, or at a toll booth. After 15 days, the cost of chasing it is added to the toll.

The Pedemontana Has No Barriers: Pay Online in 15 Days

The Pedemontana Lombarda north of Milan (the A36, A59 and A60) has no toll booths. Gantries read the plate or the toll device. Without a device, pay at pedemontana.com or in its app within 15 days of the day you drove it.

Miss that and the fine is €87 to €344, plus two points off an Italian licence. For a foreign plate, a debt-collection company follows it up. More on roads like this: free-flow tolls.

Expect a Small Rise Each January

Autostrade per l'Italia raised its tolls by 1.5% on 1 January 2026. Its 2026 rate for a car is €0.07869 a kilometre on flat sections and €0.09315 in the mountains, VAT included, and every toll is rounded to 10 cents. Other motorway companies set their own rises.

Budget €45.60 for Milan to Rome and €16.80 for the Brenner to Verona, pay by card at the exit, and settle any unpaid ticket within 15 days. Every price on this page was read from Autostrade per l'Italia's own calculator on 13 October 2026.
